Born at Liverpool, Lancashire, England, CoE. Enlisted 6 September 1914 age 29, purser. Nok father, W. C. Collins of 4 Harrington St, Liverpool. Embarked at Melbourne on 22 December 1914 per A40 Ceramic. Twice wounded at Gallipoli, 29 May 1915 and 8 August 1915. RTA June 1916 and appointment terminated 1 May 1917. Service medals:1914-1915 Star, BWM and VM. Died in 1947 at Hollywood Hospital Nedlands. Cottesloe and district Honour Rolls.
