Born at Sebastopol Victoria, Methodist. He enlisted on 15 September 1915 age 21, engine cleaner. Nok his parents, Frank and Esther Ann Cook of Auburn, cnr Glyde and Francis Sts, Mosman Park. Embarked at Fremantle 17 January 1916 per A30 Borda. On 3 September 1916, Cook was a runner carrying messages between the trenches and Battalion HQ in France when he was reported missing and later declared killed in action by a court of inquiry. A further note has him buried between Thiepval and Courcelette 4 miles NE of Albert in France. Recorded at Villers-Bretonneux Memorial, France. Service medals: 1914-15 Star, BWM and VM.
